[beeswax] Create a new table from file page is broken

Create a new table from file (/beeswax/create/import_wizard) is improperly designed.

Here are a examples of some problems:

1. The 3 steps are not hidden sometimes at the beginning or when navigating between steps
2. The page is too compacted
3. 'Choose File' should be a small '..' button like when editing a Job Design
4. The description text should be probably aligned on the right of the text boxes
5. The error messages should appear on the right of the text boxes
6. The button for going to the next step should be 'Next' and 'Previous' (like 'Create a table Manually')
7. Previous button on step 2 is either missing an optional checkbox or is useless
8. Clicking on step 1 erases everything
